Doctor Mario is Mario as he appears in the few Dr. Mario games. He is not really a seperate character, in the same way Young Link is not a seperate character from Link. The only reason Mario was dressed up as a doctor is because it would let them use the image of pills and viruses, which is a more interesting way of making a puzzle game than just using normal colored blocks.

It's just Mario playing dress up. He appears in Super Smash Bros. Melee as an unlockable character as a Mario clone. Instead of fireballs, he throws pills. Otherwise, he's slower than Mario, but stronger.
